Before you complete the form, please confirm that you are authorized to nominate candidates because you have received a letter or email communication from The Wexner Foundation inviting you to do so. Please note that you may nominate more than one individual. Self-nominations are not allowed.
Please gather the following required information about your nominee that you will need to complete the nomination form: Name, age, current email address, home or work mailing address, and at least one phone number. You will not be able to save and re-enter the form.
Your nomination is accepted only after you click submit. Once you have submitted your nomination, you will receive an email confirmation.
