2011-2012 Eligibility Forms

To download the Eligibility Form for the 2011-2012 academic year please:

1. Right click on the link below ("Download WIFP Eligibility Form")
2. Choose the option "Save Target As..." or "Save Link As..."
3. Save the PDF file on to your computer. We strongly recommend that you include your full name and date in the name of the file.
4. Open the file with Adobe Acrobat Reader or any other program that can open PDF files.
5. Once you've completed the Form, please save it in PDF format (and not XML format) and send it as an email attachment to: wifp@wexner.net, along with an updated CV and photo ("head shot").
6. Please make sure you keep a copy of these materials.
